Please download the Intake and Consent Form and complete it. Currently it will need to be printed, filled out, then either mailed to Laurin if there is sufficient time before your appointment, or even easier, you can scan it with your phone, or just take a picture of each page, and email it/them to Laurin@HeartLightJoy.com.
